Description
To do business with Qwest as a Facility-Based Competitive Local Exchange Carrier (CLEC) or Reseller, the New Customer Questionnaire must be completed by you or a representative from your organization. Your Qwest Service Manager will assist you with this process.
The New Customer Questionnaire contains instructions outlining what sections must be completed before you can begin to order products and/or services.
If the New Customer Questionnaire is not filled out correctly and/or completely, it will be returned to you, via email, for you to complete or correct the sections needed. The New Customer Questionnaire instructs you to complete some additional sections within two weeks after submitting the required sections to Qwest. If you fail to complete these other sections, it could delay your ability to order products and/or services as you may not have access to various tools and reports within Qwest.
Upon completion of the required and additional other sections, the New Customer Questionnaire should be emailed to wholesale.servicesupportteam@qwest.com. The Wholesale Service Support team member will provide you an e-mail response that your Questionnaire has been received and is in process.
The completed questionnaire is used by Qwest to determine the following:
- Determine geographical requirements
- Assign Internal Qwest CLEC or Reseller IDs and Identify Industry Codes
- Determine Qwest system requirements to support CLEC specific activity
- Obtain credit information
- Obtain service volume estimates
- Determine deposits, when applicable
- Obtain billing information
- Create Summary Bills
- Obtain Performance Assurance Plan (PAP) information
- Establish output and input requirements
- Obtain your contacts hours-of-operation and holiday schedule
New Customer CLEC Questionnaire
If you are a new Facility-Based CLEC, you must complete the New Customer CLEC Questionnaire.

If you are a new Reseller, complete the New Customer Resale Questionnaire.

Rates
Wholesale rates for this product or service, including tariff references and any applicable discounts, are provided in your current Interconnection, Resale, Commercial, or other governing agreement.
If you begin the process by filing out the New Customer Questionnaire required sections and submit it and then determine you do not want to continue, you will not be charged for the work already done by Qwest.
