The FCC rules require that a certification statement be made available at the BOC's principal place of business. FCC's Accounting Safeguards Order, In the Matter of the Telecommunications Act of 1996: Accounting Safeguards Under the Telecommunications Act of 1996, CC Docket No. 96-150, FCC 96-490 (rel. Dec. 24, 1996).:

The information made available at the principal place of business of the BOC must include a certification statement identical to the certification statement currently required to be included with all Automated Reporting and Management Information System ("ARMIS") reports. Such certification statement declares that an officer of the BOC has examined the submission and that to the best of the officer's knowledge all statements of fact contained in the submission are true and the submission is an accurate statement of the affairs of the BOC for the relevant period.

A copy of Qwest Corporation's certification statement is available for public inspection between the hours of 7:30 a.m. and 4:30 p.m., Monday through Friday (except for holidays), at Qwest Corporation's principal place of business:
