Product Description
Qwest Commercial High Speed Internet (HSI) Services and Qwest Commercial Broadband Services are commercially negotiated wholesale products provided to end-users on behalf of Competitive Local Exchange Carriers (CLECs) and Resellers.
With Qwest Commercial HSI/Broadband Services, Qwest will provide fast and reliable high speed internet connectivity and continuous dedicated access to a selected Internet Service Provider (ISP) with compatible Resale, Qwest Platform Plus (QPP), and Qwest Local Services Platform (QLSP) services. Whether you choose to provide your end users with concurrent voice and data transport over a single local loop or data only transport ('Stand Alone' service), these data transport-only solutions allow you to partner with an ISP of your choice.
Qwest Platform Plus (QPP) HSI and Broadband Services
Qwest Commercial HSI/Broadband Services are available with compatible QPP Business and QPP Residential, QPP PBX Analog (non-DID trunks), and QPP Centrex 21, QPP Centrex Plus, and QPP Centron®. Stand Alone Qwest Commercial HSI/Broadband Services (without voice) is not available with QPP.
Qwest Resale - HSI/Broadband Services
Qwest Commercial HSI/Broadband Services are available with compatible Resale services, including Resale Business and Residence local exchange access line services, also known as Plain Old Telephone Service (POTS) - Business and Residence, PBX Trunk Service, Centrex 21, Centrex Plus and Centron®, and Centrex Prime.
Stand Alone Qwest Commercial HSI/Broadband Services (without voice) are available to Reseller end users and is strictly a data line which does not include analog voice transmission capabilities or 911 service. Several data rates are available.

This residential and business product provides 3 different provisioning levels. The provisioning levels will be based on the loop qualification of the line. The provisioning levels are as follows:
3 Mbps: provides for downstream data rates up to 3 Mbps and up to 640 Kbps upstream.
5 Mbps: provides for downstream data rates up to 5 Mbps and up to 896 Kbps upstream.
7 Mbps: provides for downstream data rates up to 7 Mbps and up to 896 Kbps upstream. |

Downstream speeds refer to data rates from the network to the end-user and upstream speeds refer to data rates from the end-user to the network.
The speed(s) available at the end user premises/location are identified during the ordering process. The location may subsequently be eligible for additional speed options; provided that you will be charged a speed change charge for any speed change. Service speeds are "up to" and Qwest will provision your line at the maximum speed it qualifies for within the speed range of the service you selected. Actual speed varies depending on various factors. Qwest may limit speeds.
Grandfathered Qwest DSL® Pro products
The following products were grandfathered effective May 1, 2006. Qwest will accept only orders to disconnect or change the broadband service to a currently available service alternative.

Availability
Qwest Commercial HSI/Broadband Services are available where facilities exist throughout Qwest's 14-state local service territory.
Qwest Commercial HSI/Broadband Services are available on loops that meet specific conditions, e.g., distance from the CO. To determine if a loop qualifies, perform a "loop qualification" using Interconnect Mediated Access (IMA) or by contacting the Customer Service Inquiry and Education (CSIE).
Qwest HSI/Broadband Services may not be available in all areas or at the rates or speeds generally marketed. Availability of service depends on availability and limits of Qwest wire centers and facilities. Service will not be provided using unsuitable facilities or if provision of service creates interference with other services. Service is offered only to location(s) qualified by Qwest line qualification procedures. Some lines may not qualify for the service even if initial tests qualified such lines. Speed and availability of service are not guaranteed and may be limited by a variety of factors including but not limited to the physical condition of the line and wiring at the location, of service, phone line qualifications, computer performance/configuration, and network/Internet congestion. Service is provided on a per-line basis, and the actual throughput and connection speed of your service depends on a number of factors such as Internet traffic and congestion or bandwidth, distance of the premises from a Qwest central office, viruses or spy ware, server speed of the Web sites to which the end user connects, traffic and congestion on the premises network or corporate LAN, and Windows PC settings, in addition to the factors listed above. Uninterrupted or error-free service is not guaranteed.
If the end user moves to another location (including a move within the same building) HSI/Broadband Service is not guaranteed at the new location. The line must be re-qualified for Service at any new location and NRCs applicable to new Service will apply along with MRCs at the rate for existing customers.
Qwest Retail High Speed Internet/Broadband Service Promotions are not available.
Broadband Technology Availability Restrictions
Certain Qwest retail broadband products, primarily including
those provided to end users using Qwest Internet Protocol
(IP)/Ethernet technology (indicated by USOCs EHX++), are not
available with Qwest Commercial HSI/Broadband Service. When
you request conversion to wholesale of any existing account
that includes these unavailable broadband services, you may
request retention of broadband functionality by changing that
unavailable service to one of the available Qwest Commercial
Broadband products listed in this PCAT (providing that the
impacted end-user premises loop qualifies for service).
Terms and Conditions
Unless requesting Stand Alone (data only) service, you must also be the provider of record of the underlying voice service when requesting Qwest Commercial HSI/Broadband Service.
Qwest Commercial HSI/Broadband Services are business-to-business arrangements at just and reasonable rates provided in accordance with commercially negotiated Master Services Agreements (MSAs) between Qwest and interested Competitive Local Exchange Carrier (CLEC) customers.
You may order new Qwest Commercial Broadband Service subject to loop qualification and availability.
You may also convert existing Qwest retail end users with Qwest retail High Speed/Broadband Internet Service to Qwest Resale, or QPP service and maintain the High Speed Internet/Broadband Service, if that Service is available with Qwest Commercial High Speed Internet/Broadband Services, as provided in Product Description.
Following retail service conversion to Resale, or QPP, existing and installed Customer Premise Equipment (CPE) (e.g., modems, filters), if any, will not be maintained or changed by Qwest.
It is your responsibility to provide your end-user with CPE (modems, filters), marketing materials, installment instructions, and Tier 1 Technical Support. You must provide equipment that supports the Service and, for Qwest HSI/Broadband Service, equipment that also meets minimum system requirements and a modem from the list here. Qwest reserves the right to revise minimum requirements. You represent that you or your end user owns the CPE or otherwise have the right to use it with the Service and agree that Qwest has no responsibility or liability for it. Installing or using the Qwest HSI/Broadband Service with CPE that does not meet the minimum requirements limits support available for Service and may damage CPE, software, peripherals or data. Neither Qwest, its affiliates, suppliers, or agents, will be responsible/liable if your end user cannot access the HSI/Broadband Service, if the Service does not function correctly or at all, or if the CPE, software, peripherals, data, or equipment is damaged. This limitation of liability is in addition to and will not limit any other limitation of liability set forth in your MSA.
A qualifying Internet Service Provider (ISP) must be selected at the time you order Qwest HSI/Broadband Services. Qwest is not available for selection as an ISP and therefore you will be subject to third party provider's terms. After commencement of your service, if the ISP you selected no longer has a relationship with Qwest, or will no longer offer the service in your area, Qwest may contact you to determine a qualifying replacement provider. In this event, Qwest will allow you to select a replacement provider and will waive the associated destination change charge. Qwest may charge you a destination change charge for any other change in Internet service provider.
The following service conditions apply to Qwest HSI/Broadband Services. Qwest reserves the right to suspend or terminate the Service if any of these conditions are violated:
- The Service will not be used for high volume or excessive use or in a way that impacts Qwest network resources or Qwest's ability to provide services.
- Multiple computers/devices within a single home or office location may be connected to the modem and/or router to access the Service, but the Service may only be used at the single home or office location for which Service is provisioned by Qwest.
- Service may only be used in the U.S. and the Service will not be used to host any type of server, personal or commercial.
- Qwest may restrict use of or interrupt the Service without notice for: (i) maintenance activities; (ii) equipment, network, or facility upgrades or modifications; and (iii) to ensure the provision of acceptable service levels to all Qwest customers.
- Qwest is not responsible or liable for any Service deficiencies or interruptions caused by such events.
- The end user will not resell or distribute, transfer or assign the Service via any means including but not limited to wireless technology.
- CLEC will not permit any unauthorized use of Qwest HSI/Broadband Services. You are responsible for unauthorized and non-compliant use of the Service.
- Qwest HSI/Broadband Services cannot be used for any unlawful, abusive, or fraudulent purpose, including without limitation, using the Service in a way that: (i) interferes with Qwest's ability to provide service to Qwest customers, (ii) avoids obligation to pay for the Service, (iii) constitutes a criminal offense, (iv) gives rise to a civil liability, or (v) otherwise violates any law, order, ordinance, governmental requirement or regulation or this Agreement.
The Service may be used in a wireless network environment at end user's own risk. Wireless networking devices use public radio channels to transmit voice and data communications. Qwest cannot guarantee the security, privacy, or confidentiality of any transmissions made via such devices, and Qwest makes no assurances or warranties relating to their use. CLEC is responsible for all use of Service regardless of the source, authorized or unauthorized, of a transmission over the Service.
Unless otherwise expressly provided in your MSA, all aspects of the Service are the property of Qwest and are protected by trademark, copyright or other intellectual property laws and international treaty provisions. Qwest grants you a personal, revocable, limited, nonexclusive, nontransferable, non-assignable right and license to use the Service in accordance with the terms and conditions of this Agreement. No other license or rights are granted by Qwest or will be implied or arise by estoppel, with respect to any Service.
All use of the Services will comply with the Qwest Acceptable Use Policy (AUP). Among other things, the AUP prohibits sending unsolicited e-mail messages, including bulk commercial advertising or informational announcements (collectively, "Spam"). Qwest may immediately terminate or suspend any account which Qwest believes is transmitting or is otherwise connected with any Spam. Further, Qwest may hold you liable for Qwest's actual damages in any way arising from, or related to, any Spam transmitted by or in any way connected to you or your account, to the extent such actual damages can be reasonably calculated. If actual damages cannot be reasonably calculated, you agree to pay Qwest liquidated damages of five U.S. dollars ($5.00) for each piece of Spam transmitted from or otherwise connected with you or your account. You will not, however, be liable for actual or liquidated damages arising from Spam generated from you or your account if you establish that the Spam was sent as a result of a virus or worm or other malicious software infection and if you have taken and continue to take reasonable actions to prevent and resolve such infections.
Qwest and its affiliates, contractors, and third-party vendors, have the right to access, gather, record, and use information to manage the Agreement (including possible breaches of the Agreement), to monitor Internet connection and network performance, to access and adjust computer settings, and to perform registration, maintenance, support, and other service-quality activities related to the Service. Qwest may disclose information in accordance with our Privacy Policy and the Agreement. Qwest may disclose information to a governmental agency when permitted to do so by law, or if necessary to establish rights or obligations under this Agreement.
CLEC assumes total responsibility for use of the Service. Qwest exercises no control over and disclaims any responsibility for the content created or accessible using the Service and for actions taken on the Internet. Qwest recommends that the service not be used in high risk activities where an error could cause damage or injury.

Rate Structure
Monthly Recurring Charges (MRCs) and NonRecurring Charges (NRCs) apply to Qwest Commercial HSI/Broadband Services.
Rates
MRCs and NRCs for Qwest Commercial HSI/Broadband Service are included in this
PCAT. Applicable discounts to these MRCs and NRCs for Resale
CLECs can be found in the Qwest Commercial HSI/Broadband Service
MSA. Applicable discounts for QPP CLECs can be found
in the Qwest MSA governing the QPP service.
NRCs apply for certain Temporary Disconnection for Non-Payment
requests. See the Deny/Restore information provided in the
Ordering section of this PCAT.
The Qwest Commercial HSI/Broadband Service Change Charge (USOC REAKM) applies when you request a Speed/Destination Channel Change, and other similar changes. The NRC is $9.99. The NRC will be waived when speeds are upgraded.
Maintenance and Service NRCs are provided in your ICA or MSA, dependent upon the underlying service.
Regulations and Policies
Your Interconnection Agreement must include specific terms and conditions to purchase the underlying Resale, or QPP service. See Implementation for additional information if you need to amend your Interconnection Agreement.
Qwest Commercial HSI/Broadband Services are offered by Qwest on a non-common carrier, Title I basis and is not tariffed.

Implementation
Product Prerequisites
If you are a new CLEC and are ready to do business with Qwest, view Getting Started as a Facility-Based CLEC, and Interconnection Agreement, and Commercial Agreements.
Existing CLECs may execute an MSA for Commercial HSI/Broadband Services at Commercial Agreements.

Pre-Ordering
General pre-ordering activities are described in the Pre-Ordering
Overview.
256K speed services are not available in all areas or to
all premises where Qwest Broadband Service is available. If
256k speed services are not available, IMA will advise you
during IMA Pre-Order activity.
Loop Qualification
You must perform Loop Qualification by Working Telephone
Number (WTN) before you submit an LSR to add new Qwest HSI/Broadband
Service to an existing WTN. Loop Qualification is not necessary
or required when converting a WTN with existing Qwest HSI/Broadband
Service, unless you are making a change to the Broadband service,
e.g. speed or technology change. To enable Loop qualification,
the new WTN must be posted to Qwest billing systems. When
performing a loop qualification for a border town condition,
you may use either the dialable telephone number or the billing
telephone number. Additional information on Border Towns can
be found in the Pre-Ordering
Overview.
Loop Qualification by Address:
You must perform a Loop Qualification by address before you submit an LSR to order new Qwest HSI/Broadband Service at the same time and on the same LSR as you are requesting new line installation (N, C & T orders). When the Loop Qualification response is Provisional (additional investigation is required to see if the address will qualify) you must submit the order by populating a Y in the SCA field. Provisional response may be returned on newly built addresses. Qwest will look at nearby facilities in an attempt to provision HSI/Broadband at the new address. Note that line moves and UDC removal are not available on N & T orders, C orders adding a new line with Qwest HSI/Broadband , or stand alone service.
Loop Qualification by WTN:
When qualifying a WTN and the loop qualification response is Provisional (additional investigation is required to see if the loop could qualify), you must submit the order by populating a Y in the SCA field. This will authorize Qwest to perform a UDC removal or a line move, at no additional charge. If a line move or UDC removal is not possible, Qwest will send you a jeopardy notice for the LSR. If the pre-order loop qualification response indicates that the loop does not qualify for Qwest Commercial HSI/Broadband Services do not submit the order. Information regarding ADSL Loop Qualification and Qualification Characteristics may be found in Appendix E of the Loop Qualification and Raw Loop Data CLEC Job Aid.
Loop Qualification will return the highest speed qualified for that TN or address. The highest qualified speed returned or any lower speed is available and can be ordered by populating the appropriate USOC on the LSR.
For ISP name validation, refer to the Interconnect Service Center or your Account Manager.
End-users in specific New Mexico wire centers may request Load Coil removal per Section VIII.D of the New Mexico AFOR (Alternative Form of Regulation) final order, as approved by the New Mexico Public Regulation Commission, Qwest will remove Load Coils, at no charge, for Qwest Commercial HSI/Broadband Service loops under 18 kilofeet in length in the 18 wire centers identified in the New Mexico AFOR. For specific wire centers and Load Coil removal ordering information, refer to New Mexico Load Coil removal.

Loop qualification requests for 20 or more WTNs should be emailed to Qwest. Information regarding the process is described in Appendix E of the Loop Qualification and Raw Loop Data CLEC Job Aid.
If you are not an IMA user you should contact the CSIE at 866-434-2555 to request loop qualification by WTN or address.
Requirements for pre-ordering are described in Local Service Ordering Guidelines (LSOG) Pre-Order.
Qwest Host Volume Discount Program arrangements are not available under this agreement. This arrangement is indicated by the presence of a HSI/Broadband USOC followed by a 'DVDP' Field Identifier (FID) on the CSR. When you request conversion of an existing Retail service with this HSI/Broadband arrangement, Qwest will remove the DVDP FID from the account while keeping end-user HSI/Broadband functionality in place. This process will take down the Volume Discount Program's Host ISP, therefore an ISP Host change is required for the conversion process.
When ordering Stand Alone Qwest HSI/Broadband Service you must perform a Loop Qualification by address. You must qualify for a minimum of 1.5 Mbps speed. The TNS field on the LSR will need to be populated with either a reserve TN or a placeholder TN (the TN will be changed by Qwest to a data TN and provided upon Firm Order Confirmation (FOC)).
When the Loop Qualification response is Out of Capacity, do not submit a request for new Qwest Stand Alone HSI/Broadband Service.

Ordering
General ordering activities are described in the Ordering Overview.
Service interval guidelines are found in the Service Interval Guide (SIG).
Orders should be placed using the IMA Graphical User Interface (GUI), the IMA Extensible Markup Language (XML), or faxed to 888-796-9089.
Qwest HSI/Broadband Service requests are submitted using the following LSOG forms:
- Local Service Request (LSR)
- End User (EU)
- Resale Service (RS), if applicable
- Port Services (PS), if applicable
- Centrex Resale Service (CRS)
- Directory Listing (DL), if applicable
Note: QPP Business and Residential requests submitted using IMA Version 17.0 or higher will use the Port Service (PS) form. Previous IMA versions and Qwest Commercial HSI/Broadband Service with other products will continue to use the form that is currently required for that product.
Follow the field entry requirements described in the LSOG.
When changing the TN on a line with Qwest Commercial HSI/Broadband Service, the new telephone number must be a telephone number that is currently not a working number in a Qwest switch. The number cannot be "swapped" for another line. The Qwest Commercial HSI/Broadband Service would need to be removed from the line(s) before a TN swap could take place.
When converting an existing resale or retail account with HSI/Broadband Service, follow the LSOG order requirements for the underlying service to which the service is being converted.
On the RS or PS form in the MEGACENTNM field include the Mega Central Name that matches the valid Mega Central Circuit ID.
The appropriate Qwest Commercial HSI/Broadband Service USOCs must be indicated in the FEATURE section of the RS, PS, or CRS form for Qwest Commercial HSI/Broadband Service requests.
Choose the appropriate service/line USOCs when ordering Stand Alone Qwest Commercial HSI/Broadband Service, either GFB for Business service or GFR for Residential service. Stand Alone Qwest Commercial HSI/Broadband Service requires a data TN assignment. When ordering Stand Alone service, the TNS field on the RS form may be populated with either a reserve TN or a placeholder TN. When the service request is processed, Qwest will change the TN to a data TN.
When changing from a POTS line with Qwest Commercial HSI/Broadband Service to a Stand Alone Qwest Commercial HSI/Broadband Service line, the TN will change from a voice TN to a data TN. Feature Activity (FA) Types:

When changing from Stand Alone Qwest Commercial HSI/Broadband Service line to a POTS line with Qwest Commercial HSI/Broadband Service, the TN will change from a data TN to a voice TN. Feature Activity (FA) Types:

The PIC and LPIC fields on the RS form must be populated with NONE for Stand Alone Qwest Commercial HSI/Broadband Service.
The data TN on Stand Alone Qwest Commercial HSI/Broadband Service will change when moving service to a new location.
To request a line move or UDC removal, enter 'Y' in the SCA field of the LSR form. Without an entry in the SCA field, you have not authorized a line move or UDC removal. If the loop qualification response is Provisional and you have not authorized a line move or UDC removal, the order will be rejected. If the loop does not qualify for Qwest Commercial HSI/Broadband Service, IMA will not accept the request and will return an error message.
If at any time, after you receive the Firm Order Confirmation (FOC) and prior to the scheduled due date, Qwest determines the qualification was a false positive (when loop qualification is successful but High-Speed Internet/Broadband cannot be provisioned on the line without a line move or UDC removal), Qwest will make every attempt to provision the High-Speed Internet/Broadband request, including a line move or UDC removal, within the original scheduled DD without requiring a supplemental LSR. In the event the line move or UDC removal work cannot be completed within the original scheduled DD, the CLEC will be notified by a jeopardy notice and required to submit a SUP, within 4 business hours, with a Y in the SCA field authorizing the standard interval. All Carrierless Amplitude Protocol (CAP) based DSL service has been grandparented and is no longer available for conversion or new installation.
When issuing a T&F to move your end user they may keep the existing TN with Qwest High-Speed Internet/Broadband service, however, Dual service is not allowed.

Partial migrations, conversions, consolidations, and deconsolidation
To minimize or avoid extended broadband service interruption on the due date for these order activities, LSRs for partial migration/conversion and consolidation/deconsolidation accounts where broadband is retained should include the following information:

Deny/Restore
When Temporary Disconnection for Non-Payment is requested,
your end-user’s Qwest Commercial HSI/Broadband Service
will be interrupted and will not allow any Internet activity.
There is no specific NRC to deny broadband service,
however other service charges may apply dependent upon the
underlying voice service. A $25.00 NRC will be applied when
service is restored. Additional information on Temporary Disconnection
for Non-Payment is available in Temporary
Disconnection for Non-Payment/Restore PCAT.
To simultaneously
deny both voice service and broadband service, submit to Qwest
an LSR for temporary disconnection of the voice service. With
this order, both voice and broadband service will be denied.
To restore both voice and broadband service, submit an LSR
for restoral of the voice service as detailed in the Temporary
Disconnection for Non-Payment/Restore PCAT.
When requesting temporary disconnection or restoral of only
the Qwest Commercial HSI/Broadband Service submit to Qwest
one LSR indicating the following:

NOTE: Requests denying or restoring Qwest Commercial HSI/Broadband Service can not be combined with any other types of order activity.
NOTE: On Deny LSR cancellations or changes, once Qwest has received an LSR and has issued an order to deny Qwest Commercial HSI/Broadband Service, any supplemental LSR that you submit to cancel or change this deny order will not be processed by downstream broadband systems. The broadband service will be denied on the original requested Due Date. All supplemental LSRs must follow the guidelines in both the Ordering Overview as well as the Temporary Disconnection for Non-Payment/Restore PCAT.
To cancel or change a pending order for service denial, you must: 1) Cancel the LSR; 2) Contact the CSIE to request broadband service restoral ; and 3) Submit a new LSR, if necessary.
Seasonal Service for Qwest High-Speed Internet service is offered to CLECs and Resellers in all states except Idaho-North, Oregon and Washington. When you request this service for your residential end users Qwest Commercial high-speed Internet service will be placed on Seasonal Service status.
High-Speed Internet/Broadband Seasonal Service:
- Is not available to business class end users.
- Is available on all High-Speed Internet/Broadband speeds including Standalone High-Speed Internet/Broadband service.
- Has a minimum 30 day suspension period.
- Is available at a monthly recurring rate of $9.99 and a non recurring charge of $14.99 minus any applicable Commercial Agreement discount. Normal High-Speed Internet/Broadband charges will not apply during the Seasonal Service period.
- Once Seasonal Service has been restored it cannot be placed on Seasonal Service again for a 90 day period.

Information regarding directory listings is available in the White Pages Directory Listings PCAT. When Stand Alone Qwest Commercial HSI/Broadband Service is the only service on an account, a directory listing is required but the listing will not be sent to the Qwest Listings database.

Provisioning and Installation
General provisioning and installation information is identified in the Provisioning and Installation Overview.
When you submit an LSR to order new Qwest Commercial HSI/Broadband Service at the same time and on the same LSR as you are requesting new line installation (N & T), note that the new line installation interval or the HSI/Broadband installation interval may apply, whichever is longer. For service intervals refer to the Service Interval Guide (SIG).
In the event that Qwest determines that the data portion of the request cannot be provisioned with the voice after an FOC has been issued, Qwest will send a jeopardy notice to the CLEC. The jeopardy notification will provide you with information on how to proceed. The jeopardy process is described in the Provisioning and Installation Overview.
Upon receipt of an accurate and complete LSR including authorization of a line move or UDC removal, you will receive a Firm Order Confirmation (FOC) based on the standard interval found in the SIG. If at any time after you receive the FOC, and prior to the scheduled due date, Qwest determines that a line move or UDC removal is required, Qwest will apply the line move or UDC removal interval and will re-FOC your LSR reflecting the new scheduled due date. The interval will begin on the date Qwest determines that a line move or UDC removal is necessary and a supplemental LSR is not required.
If at any time after you receive the FOC, and prior to the scheduled due date, Qwest determines that a line move or UDC removal cannot be accomplished to Loop Qualify, you will receive a jeopardy notification via IMA. The jeopardy notification will provide you information on how to proceed.
Loop facilities must be available when ordering Qwest Stand Alone Qwest Commercial HSI/Broadband Service.

Maintenance and Repair
General maintenance and repair activities are described in the Maintenance and Repair Overview.
Charges apply for certain maintenance, trouble isolation, and support services and if a technician is dispatched. Charges may be per technician, may vary depending on when services are performed (e.g., time of day and weekday, holiday or weekend), and may include a minimum charge regardless of the actual number of hours worked. Qwest will notify you of any applicable charges in advance of you incurring such charges. If you report trouble, you must pay a maintenance of service charge if the trouble is not found in Qwest facilities (no charge if Qwest later finds the trouble was in Qwest facilities) or Qwest equipment or is found in customer equipment/systems or Equipment. A dispatch charge also applies if: (A) Customer requests a service date change but fails to notify Qwest before the service date and Qwest technician is dispatched on the service date (will have to pay dispatch charge and Qwest will change the service date) or (B) Qwest technician dispatched for maintenance of service and no trouble is found in Qwest facilities (applies each time this happens).
Data Backer and the Home/Office Network Backer are not available with Qwest Commercial HSI/Broadband Service.
If your end-user's HSI/Broadband Service is disconnected by Qwest in error during conversion activity and you have submitted an accurate and complete LSR requesting conversion of existing Qwest HSI/Broadband Service which results in disconnection of the service, Qwest will follow HSI/Broadband repair intervals to reestablish the service. Additional information on reporting this condition is available in the Recent Service Order Activity subsection of the Maintenance and Repair Overview. In the event you and the Technical Support Center determine that HSI/Broadband speed needs to be permanently lowered in order for the service to work properly, you may be requested to replace your current HSI/Broadband USOC with a different USOC or one of the following repair USOCs:

NOTE: The word "repair" may be returned in the IMA PreOrder Loop Qualification if a repair USOC is present or if the maximum speed available for that service address is one of these speeds.
Please follow the steps below to update your existing USOC:
- An LSR will be required to change the speed of the service. Use a Feature Activity (FA) of "D" for the old USOC and an FA of "N" for the new USOC.
- Mark the LSR for Manual Handling and add a REMARK stating "due to a repair issue, we need to lower the HSI/Broadband speed". Include the Technical Support Center repair ticket number in the remark.
The service order for this type of repair issue will have the same Business day due date and the speed change charge will be waived.
NOTE: If IMA will not accept the USOC to change the speed, enter the USOC in the Remarks section of the LSR.
When a repair issue is identified on CAP technology HSI/Broadband , there may be situations that require a change to discrete multitone (DMT) technology. When repair determines that the CAP service will continue to experience service impacting issues you will have the following options:
- Change the CAP service to DMT
- Disconnect the CAP service
- Disconnect the CAP service and change to POTS service
To make the desired change, you are required to submit a complete and accurate LSR.
For copper retirement notifications when CAP service is involved, the same three options listed above are available.

Billing
On a monthly basis, Qwest will provide you with billing information that will provide summary account information as well as end-user sub-account information. Detailed information regarding the Customer Records and Information System (CRIS) Summary Bill, Inquiry and Disputes is described in Billing Information - Customer Records and Information System (CRIS).
Stand Alone HSI/Broadband Service is not billed on the same account as Centrex or PBX trunks.
MRCs and NRCs billed in advance and usage and governmental charges, unless you are tax exempt, are billed in arrears. You will pay all billed charges by the due date set forth in your billing statement, as well as any Late Charge. You will be charged for each additional copy of your bill.

Frequently Asked Questions (FAQs)
When Qwest Commercial HSI/Broadband Service is provisioned with compatible QPP or Resale services, does the end-user receive a bill from Qwest?
The customer of record for Qwest Commercial HSI/Broadband Service when provided with QPP or Resale services will be the CLEC. There will only be one "customer of record" for both the underlying line and commercial HSI/Broadband service, and one bill will be sent for both products. Additionally, Qwest will not accept requests for Commercial HSI/Broadband service from the end-user or any provider other than the CLEC customer of record.
Why aren't Qwest HSI/Broadband Services tariffed?
Effective November 16, 2005 companies providing high-speed Internet/Broadband access have the choice of continuing to provide terms and conditions that govern the service under FCC Tariff, or may use a Commercial Agreement. Qwest has made the decision to move, effective January 28, 2006 to a Commercial Agreement to govern Qwest High-Speed Internet/Broadband service.
