Change Requests (CRs)

It is easy to create a change request. Download the CLEC Change Request Form with Instructions below and follow those instructions to complete your form. Once completed, email your completed change request to the appropriate CMP Manager (Product/Process or Systems).

Also, if you would like to review submitted change requests and their status, you may download the change request log or individual change requests from the pages listed below.
