Description
Qwest Business Line Plus and Qwest Utility Line are a package
of features available to business customers in conjunction
with an individual or additional flat rate business line.
Business customers subscribing to the package are entitled
to the following features:
- Flat Rate Business Line (1FB)
- Call Forwarding Variable (ESM)
- Call Transfer (EO3) also has the functions of Three Way
Conferencing. However, Three Way Calling (ESC) cannot be
sold separately with Call Transfer on the same telephone
number.
Qwest Business Line Plus package has three options for service:
- Month to month service
- One year contracted service
- Two year contracted service
Qwest Utility Line provides an additional line. This line
can only be added in conjunction with a contracted Qwest Business
Line Plus. The Qwest Utility Line can only be provided on
a one-to-one basis (maximum of one Qwest Utility Line with
each Qwest Business Line Plus package). No additional services,
such as features or listings, are allowed with this line.
Contracts do not have to be signed, but Termination Liability
Assessment (TLA) may apply if a contracted package is removed
prior to the expiration of the contract term.
Agreement Terms are as follows:
- If Qwest terminates the service(s) for cause, or if the
customer terminates the service (s) in whole or in part
without cause, the customer will pay termination charges
calculated at 15% of the remaining value of the terminated
service over the remaining term (including any services
added after the date of the acknowledgement). The customer
must pay all recurring and nonrecurring charges for the
services being terminated through the date of termination.
- Additional services may be added to the customers
commitment, at the rate for services permitted hereunder,
and the term with respect to those similar tariffs from
Qwest at the same time as the termination of services which
has a price equal to 115% of the then remaining amount of
the services being terminated.

Availability
Qwest Business Line Plus package is available:
- In all states except Oregon Rate Zone 3
- With flat business basic exchange lines including Megabit
- In 5ESS® and 5RSM (5ESS remote switch),
DMS-10, DMS-100 and RSC (DMS remote switch), Ericsson
(AXE) and ISDN central office types (a remote is a switch
that is away from its host or control office)
- Wherever Call Transfer and Call Forwarding Variable are
available
Qwest Business Line Plus package is not available:
- With other packages, e.g., Custom Calling Service, CustomChoice®
Qwest Utility Line is available:
- Wherever Qwest Business Line Plus is available
- On end-user accounts with a one or two year contracted
Qwest Business Line Plus package
- On a one to one basis with Qwest Business Line Plus;
the end-user must have one Qwest Business Line Plus line
for each Qwest Utility Line
Qwest Utility Line is not available:
- On month to month Business Line Plus

specific rate sheet in your Interconnection or Resale Agreement.

than ACT = W – Conversion As Is, must be marked for
manual handling. Please add Remarks stating that you
are Adding, Moving or Changing the package. Feature Activity Types (Action Codes)

Qwest Business Line Plus is identified by the following USOCs:

NLUDG Two Year Contract
Contracts are state specific and term specific; e.g., to
offer the one or two year term, a one or two year contract
is needed. Multiple Qwest Business Line Plus packages can
be billed on the same account. Contracted packages on the
same account must have the same term all one year or
all two year. A month-to-month package may be billed with
a contracted package.
