Qwest Choice Home™ Plus Packages V4.0

Product Description

Qwest Choice Home Plus is a package which includes a flat rated residential line and a choice of any or all of the features included in the Features Chart in the Ordering Section of this document.

2-Line Qwest Choice Home Plus Package offers the same elements as the 1-Line package but with an additional flat rated residential line. Features added to the second line of the package will be billed at the monthly Tariffs/Catalogs/Price Lists rate, less any applicable Wholesale discount.

Availability

Qwest Choice Home Plus are available where facilities exist throughout Qwest's 14-state local service territory except in Idaho-North (ID-N).

Compatibility & Restrictions

For specific feature compatibility and restrictions, see the individual feature Product Catalogs (PCATs).

Qwest Choice Home Plus Packages can not be provided on the same line with other packages.

Qwest Choice Home Plus Packages are compatible with residential line Universal Service Order Codes (USOCs) 1FR, AFH, HFR, AFL, RUA.

2-Line Qwest Choice Home Plus Package must always include two flat residence lines. Both of the end-user lines must be established on the same account. All features associated with the package must be on the same telephone number as the package USOC (PGO2P).

Directory Assistance (DA) Call Allowance is a feature only available with Qwest Choice Home or Qwest Choice Home Plus.

Area Wide Plan is an optional calling plan available with Qwest Choice Home Plus. It is not included as one of the package features. This calling plan is available in designated areas in SD and WY border towns. For additional information and availability see the state specific Tariffs/Catalogs/Price Lists

Pricing

Rate Structure

Qwest Choice Home Plus Packages may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.

There is no charge to add or remove features on an existing package. There is no charge to add a package to an existing line. There are no nonrecurring charges to add ala-carte features if listed in the Feature Chart in the Ordering Section of this document. 2-Line Qwest Choice Home Plus package features must be on the same telephone number as the package USOC (PGO2P). Termination Liability Assessment (TLA) will be waived on other Plain Old Telephone Services (POTS) packages converting to Qwest Choice Packages.

Retail rates for these packages can be found in the state specific Tariffs/Catalogs/Price Lists.

Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.

Ordering

Ordering Rules

General pre-ordering activities are described in the Pre-Ordering Overview.

Linebacker is an optional service with no additional charge and will automatically be added when ordering the Qwest Choice Plus Packages to avoid Trouble Isolation Charges (TIC).

Last Call Return and Three Way Calling USOCs will automatically be added with the packages unless there is a corresponding Pay Per Use (PPU) Blocking USOC on the same telephone number.

Directory Assistance Call Allowance is an optional service with no additional charge and will be added automatically when ordering the Qwest Choice Home Plus Packages to avoid billing on the first 6 calls.

2-Line Packages

When adding, changing, removing or converting a Qwest Choice Home Plus Package, you must provide the USOC PGO2N. PGO2N enables Qwest to correctly identify and bill the package elements on 2-Line Qwest Choice Home package and is associated with the second line.
