Qwest Choice Home is a package which includes a flat rated residential line and up to 3 feature choices. For a complete list of available features see: Qwest Choice Home Features.
2-Line Qwest Choice Home Package offers the same elements as the 1-Line package but with an additional flat rated residential line.
Additional features (ala-carte), above the 3 selected in the 1-Line and 2-Line packages, may be added at the monthly Tariffs/Catalogs/Price Lists rate, less any applicable Wholesale discount.
Qwest Choice Home is available where facilities exist throughout Qwest's 14-state local service territory except in Idaho-North (ID-N).

Qwest Choice Home Packages can not be provided on the same line with other packages.
Qwest Choice Home Packages are compatible with residential line Universal Service Order Codes (USOCs) 1FR, AFH, HFR, AFL, RUA.
2-Line Qwest Choice Home Package must always include two flat residence lines. Both of the end-user lines must be established on the same account. The three features included in the package rate must be on the same telephone number as the package USOC (PGO2H).
Directory Assistance (DA) Call Allowance is a feature only available with Qwest Choice Home or Qwest Choice Home Plus and is only available as one of the three feature choices.
Area Wide Plan is an optional calling plan available with Qwest Choice Home. It is not included as one of the package features. This calling plan is available in designated areas in SD and WY border towns. For additional information and availability see the state specific Tariffs/Catalogs/Price Lists
Qwest Choice Home Packages may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.
There is no charge to add or remove features on an existing package. There is no charge to add a package to an existing line. There are no nonrecurring charges to add ala-carte features if listed in the Feature Chart in the Ordering Section of this document. 2-Line Qwest Choice Home package features must be on the same telephone number as the package USOC (PGO2H), ala-carte features may be added to either line. Termination Liability Assessment (TLA) will be waived on other Plain Old Telephone Services (POTS) packages converting to Qwest Choice Packages.

Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.
Additional features, over the three included in this package, will be billed at the monthly tariff rate, less any applicable Wholesale discount: see Tariffs/Catalogs/Price Lists.

2-Line Packages
When adding, changing, removing or converting a Qwest Choice Home Package, you must provide the USOC PGO2N. PGO2N enables Qwest to correctly identify and bill the package elements on 2-Line Qwest Choice Home package and is associated with the second line.
The valid Feature Activity Types are:
D = Disconnect
N = Add
