Product Description

Qwest Choice Business™ Add-A-Line is a package that allows you to order up to two additional lines for every Qwest Choice Business or Qwest Choice Business Plus package (per location), allowing you to create virtual multi-line packages. Each Qwest Choice Business Add-A-Line includes the business line and 1 feature. Additional “add on” features are available at the Tariffs/Catalogs/Price Lists rate after the initial feature selection has been made.

Availability
Qwest Choice Business packages are available where facilities exist throughout Qwest’s 14-state local service territory except in Idaho-North (ID-N) and in Rate Group 3 in Oregon (OR).

Compatibility & Restrictions
For specific feature compatibility and restrictions, see the individual Resale Features Product Catalogs (PCATs).

Qwest Choice Business Add-A-Line is not a stand alone package and must be associated with Qwest Choice Business or Qwest Choice Business Plus packages.

Qwest Choice Business Add-A-Line packages are compatible with business line Universal Service Order Codes (USOCs) 1FB, AFK, 1FA, AFV, HFB.

Pricing

Rate Structure
Qwest Choice Business Plus packages may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.

Qwest Choice Business packages may have a recurring and nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.

Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.

Additional features (over the 1 included in the package) will be billed at the Tariffs/Catalogs/Price Lists rate, minus applicable Wholesale discounts: see Tariffs/Catalogs/Price Lists.

Feature Activity Types (Action Codes)
The valid Feature Activity Types are:

• D = Disconnect
• N = Add

NOTE: When changing from one feature to another, use N and D Feature Activity Types.

The following USOCs are required when ordering Qwest Choice Business Plus package:

Select one of the following features to be included in the Qwest Choice Business Add-A-Line package price. Other features may also be included at an additional monthly rate.
