Qwest Choice Business is a package, which includes a flat rated business line, Universal Single Call Telecommunications Answering and Repair™ (UniStar™), which is a Qwest line maintenance plan, and a choice of 5 features. Additional "Add On" Features are available at a discounted rate, after the initial 5 feature selection has been made.
2-Line Qwest Choice Business package offers the same features but with an additional flat rated business line. The second line in the 2-Line package also includes a choice of hunting or a call forwarding feature. The 5 initial features must all be on one line, however, "Add On" features may be added to either line.

Qwest Choice Business packages are available where facilities exist throughout Qwest's 14-state local service territory except in Idaho-North (ID-N).

For specific feature compatibility and restrictions, see the individual feature Product Catalogs (PCAT's).
Qwest Choice Business packages can not be provided on the same line with other packages.
Qwest Choice Business packages are compatible with business line Universal Service Order Codes (USOCs) 1FB, AFK, 1FA, AFV, HFB.
2-Line Choice Business packages must always include two flat business lines. Both of the end-user lines must be established on the same account.
Directory Assistance (DA) Call Allowance is a feature only available with Qwest Choice Home or Business Packages and is only available as one of the five initial feature choices.
Business Voice Messaging Service is not discounted on the second line in the 2-line package.
Qwest Choice Business packages may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.
There is no charge to add or remove features on an existing package. There is no charge to add a package and included features to an existing line. TLA is waived when an account is converted from another package to Qwest Choice.

Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.
Monthly rates for packages that exceed the 5 choices will be determined by the features chosen. The most expensive 5 features will be included in the package rate with no additional charge. For example, if 5 initial features are chosen and then several additional "Add On" features are chosen and Voice Mail and DA Call Allowance are two of the choices, the Voice Mail and the DA Call Allowance would automatically become part of the package since they are the most expensive features. Additional features (over the 5 included in the package) will be billed at $4.00 or the Tariffs/Catalogs/Price Lists rate, whichever is less expensive, minus applicable Wholesale discounts: see Tariffs/Catalogs/Price Lists.

2-Line Packages
When adding, changing, removing or converting a Qwest Choice Business package, you must provide the USOC PGO2L. PGO2L enables Qwest to correctly identify and bill the package elements on 2-Line Qwest Choice Business package and is associated with the second line. If you are converting an existing package to Qwest Choice Business, remove the second line USOC of PGO2N and replace it with PGO2L.
