Product Description

CustomChoice® Packages - Residence allows end-users to choose flat lines and features from a menu of Custom Calling and Custom Local Area Signaling Services (CLASS™) features for a monthly rate. There are three different packages available for your end-user; CustomChoice - Residence, Additional Line CustomChoice - Residence and 2-Line CustomChoice - Residence.

CustomChoice - Residence allows end-users to choose one flat line and features. End users are also eligible for discounted rates on optional products where these individual products are available.

Additional Line CustomChoice - Residence is identical to CustomChoice - Residence except that it is valid only on additional lines with the USOC 999AL.

2-Line CustomChoice - Residence allows end-users to choose two flat lines and features. End users are also eligible for discounted rates on optional products on the first line where these individual products are available. Standard rates apply to features and optional products on the second line in the package.

Availability
CustomChoice - Residence and Additional Line CustomChoice - Residence are available where CLASS is available.

CustomChoice - Residence and Additional Line CustomChoice - Residence are allowed:

  • Only on flat residence lines, including Qwest High-Speed Internet™.
  • With Total Package and Everywhere Line.

NOTE: In a DMS-100 central office (CO) switch type, a limited number of features is allowed per line.

CustomChoice - Residence and Additional Line CustomChoice - Residence offerings are not allowed on Foreign Exchange lines.

2-Line CustomChoice - Residence is available wherever CLASS is available in AZ, CO, IA, MN, NE, NM, OR, SD, UT, WA.

2-Line CustomChoice - Residence:

  • Must always include two flat residence lines billed on the same account.
  • Is allowed on Qwest High-Speed Internet.
  • Cannot have on the same line with other packages, except: may have with Total Package and Everywhere Line and may have an Additional Line package on the second line.

Pricing

Rate Structure
CustomChoice - Residence has a recurring charge for the package, except in ID-S that bills the difference between the line rate and the package rate. There are no nonrecurring charges for the features included in the package. There may be a line nonrecurring charge. Recurring and nonrecurring charges are based on state specific Tariffs/Catalogs/Price Lists.

Additional Line CustomChoice - Residence is identical to CustomChoice - Residence with the exception that it has a reduced recurring charge for the package, except in ID-S that bills the difference between the line rate and the package rate.

2-Line CustomChoice - Residence has a recurring charge for the package and includes two flat residence lines in that rate. There are no nonrecurring charges for the features included in the package. There may be a line nonrecurring charge. Recurring and nonrecurring charges are based on state specific Tariffs/Catalogs/Price Lists.

Two-Line Packages

When changing, removing or converting 2-Line CustomChoice - Residence, you must provide the USOC PGO2N. PGO2N is not used for billing or provisioning the line; it enables Qwest to correctly identify and bill the package elements on the account. The following ordering rules illustrate examples of when the USOC is required.

Feature Activity Types (Action Codes)
The valid Feature Activity Types are:

  • D = Disconnect
  • N = Add

NOTE: When changing from one feature to another, use N and D Feature Activity Types.
