The Qwest Choice TV & OnLine Sixth Anniversary Sweepstakes begins at 12 a.m. (CDT) on 8/1/2001 and ends at 5:00 p.m. (CDT) on 8/31/2001. Qwest Choice TV & OnLine sponsors the Sweepstakes.

ELIGIBILITY

This Sweepstakes is open only to lawful U.S. residents, 18 years of age or older who reside in the state of Nebraska. This Sweepstakes is void in NY, FL and where prohibited. Employees of Qwest Choice TV & OnLine, Qwest Communications International Inc., their affiliates, advertising and promotional agencies and their immediate families or those living in the same household are not eligible. All federal, state and local laws and regulations apply.

HOW TO ENTER

There are two ways to enter the Sweepstakes. Method 1: Visit the Qwest Choice TV & OnLine office listed in the direct mail advertisement. Hand print your complete name, address and day & evening phone numbers on the entry form located at the Qwest Choice TV & OnLine office listed and drop it into the entry box located in the Qwest Choice TV & OnLine office. Method 2: Hand print your complete name, address and day & evening phone numbers on the entry form printed on any Qwest Choice TV & OnLine sixth anniversary direct mail piece. Mail it to: Qwest Choice TV & OnLine | 2441 S. 130th Circle | Omaha, NE 68144 attn: Sixth Anniversary Sweepstakes. Limit one entry per household. Multiple entries received from one household will void chances of winning. Mail-in entries must be received no later than 11:59 p.m. (CDT) on 8/31/2001. Dropped off entries must be received no later than 11:59 p.m. (CDT) on 8/31/2001. Sponsor is not responsible for lost, late, misdirected, mutilated, incomplete, illegible, postage-due mail or stolen entries. All entries become the property of Qwest Choice TV & OnLine and will not be returned. Incomplete entries will not be considered.

PRIZE(S)

1 Grand Prize — One (1) year free Qwest Choice TV Full Choice® cable TV service, and free standard video installation. Retail value of $357.35. The random drawing(s) will be held on or about 9/7/01 by a representative of Qwest Choice TV & OnLine whose decisions are final, binding and non-appealable on all matters relating to this Sweepstakes.

Prize winner(s) will be notified by (telephone and/or mail) on or about 9/8/01 and will be required to produce I.D., sign an affidavit of eligibility, waiver of liability, publicity release form and a 1099 which must be completed and returned within 30 business days of (mailing/telephone notification) date or prize will be forfeited and awarded to an alternate winner.

Winners agree that receiving a prize grants permission to Qwest Choice TV & OnLine to use the winner's name, and/or likeness in advertisements, publicity, and other communications without additional compensation. Prize is not transferable or redeemable for cash and prize substitutions will not be allowed, except at discretion of Sponsor. Any taxes, fees, or incidental costs including but not limited to income taxes and travel to and from Qwest Choice TV & OnLine to complete necessary forms which may arise and be due or asserted on account of the prize shall be the sole responsibility of the winner. A 1099 tax form will be the responsibility of the winner. Odds of winning depend on the number of entries received. Limit one prize per person/household.

GENERAL CONDITIONS

By entering, participants agree to these Official Rules and the decisions of the Sponsors and the judging agencies, which shall be final in all matters relating to the Sweepstakes. Qwest Choice TV & OnLine officers, directors, agents and employees are not responsible or liable for: (1) incorrectly sent or received mailed entries; (2) errors or delays in sending entries; (3) any events beyond the control of the Sponsors, which may cause the Sweepstakes to be stopped; (4) any losses or damages caused by the acceptance or use of the prize or from participation in the Sweepstakes including without limitations personal injury or death, or (5) any printing or typographical errors in any materials relating to the Sweepstakes. The Sponsors and their independent judging organizations may stop you from participating in the Sweepstakes if you repeatedly ignore the Sweepstakes Official Rules or act: (a) in a manner they determine to be not fair; (b) with an intent to annoy, threaten or harass any other entrant or the Sponsors; or (c) in any other disruptive manner. Sponsors reserves the right to cancel or modify this Sweepstakes and entrants waive any claims against Qwest Choice TV & OnLine at any time without liability with respect to such cancellation or modification should any causes beyond the control of the Sponsors and their agencies affect the Sweepstakes.
