QWEST HIGH SPEED INTERNET® TECHNOLGY 2009 SWEEPSTAKES OFFICIAL RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING: The Qwest High Speed Internet Technology Sweepstakes (the "Sweepstakes") starts November 9, 2009 at 6:00 AM. Pacific Standard Time (PST) and ends December 19, 2009 at 10:00 PM Pacific Standard Time (PST) ("Promotion Period"). Sponsored by Qwest Corporation.
ELIGIBILITY: The Sweepstakes is open to legal residents of AZ, CO, IA, ID, MN, MT, ND, NE, NM, OR, SD, UT, WA and WY who are eighteen (18) years or older at the time of entry. Employees, officers and directors of Qwest, Marketing Lab, and their affiliates, advertising and promotional agencies and their immediate families (spouse, siblings, parents and children and their spouses) and members of their immediate households are not eligible. The Sweepstakes is subject to all applicable federal, state, local laws and regulations.
TWO WAYS TO PARTICIPATE:
ENTRY METHOD 1:
During the Promotion Period, eligible individuals who subscribe to new Qwest High Speed Internet® 1.5 speed or higher service with Windows Live™ will automatically be entered into the sweepstakes. The service must be ordered by December 19, 2009 and be activated by December 28, 2009 in order to be eliegible.
ENTRY METHOD 2:
All other eligible entrants (non-Qwest customers, Qwest customers who do not wish to subscribe to Qwest High-Speed Internet service, and Qwest customers who already are Qwest High-Speed Internet customers) may receive one sweepstakes entry by submitting a 3” x 5” card with the following information: full name (first and last), telephone number and full address (including city, state and zip code). All cards must be postmarked by December 19, 2009 and received by December 28, 2009.

Limit one entry per person/per household or email address during the Promotion Period, regardless of the method of entry. In the case of multiple entries, only the first entry will be accepted.
WINNERS SELECTION: The potential winners of the Sweepstakes (the "Winners") will be selected by random drawing on or about January 7, 2010 from among all eligible entries received to determine a total of five (5) winners by the Sweepstakes Administrator, whose decisions on all matters pertaining to the Sweepstakes are final. Odds of winning depend on the total number of eligible entries received.. Limit one (1) Prize per person, family or household.

One (1) 3rd PLACE PRIZE WILL BE AWARDED. Winner will receive One (1) Zune HF 16 GB MP3 player, Canon Powershot SD790 digital camera APPROXIMATE RETAIL VALUE OF PRIZE: $410.00
One (1) 4th PLACE PRIZE WILL BE AWARDED. Winner will receive One (1) Zune HF 16 GB MP3 player, APPROXIMATE RETAIL VALUE OF PRIZE: $220.00
One (1) 5th PLACE PRIZE WILL BE AWARDED. Winner will receive One (1) Canon Powershot SD790 digital camera APPROXIMATE RETAIL VALUE OF PRIZE: $200.00
APPROXIMATE RETAIL VALUE OF SWEEPSTAKES: $4,909.00
NOTIFICATON: Winners will be notified by mail, telephone and/or e-mail on or before January 14th. If permitted by law, winners may be required to complete and return an Affidavit or Declaration of Eligibility and a Liability/Publicity Release within 5 business days. Failure to timely return the signed documents or return of any prize notification as undeliverable will cause prize to be forfeited and awarded to an alternative winner, if possible. In the event that a potential winner cannot be notified within five (5) days of the drawing, or declines his prize an alternative winner will be selected.
PRIZE RESTRICTIONS: Winners are responsible for payment of any costs not specified relating to the acceptance and use of a prize including all applicable taxes (see Taxes section below) on prizes. No cash redemption or transfer of prize. No prize substitution or transfer of prize permitted, except by the Sponsor due to prize unavailability. If prize is unavailable, Sponsor’s liability is limited to replace with a prize of equal value. Actual prize may differ from that pictured in advertising.
TAXES: A Prize winner may be required to provide certain tax-related documents to Qwest before receiving a prize. Each Prize winner is solely responsible for any and all federal, state and local income taxes associated with his or her Prize. Qwest is not responsible for and will not pay or in any way compensate Prize winners for taxes associated with winning a Prize in this contest. If required by law, Qwest will issue Form 1099-MISC to a Prize winner reporting the taxable retail value of his or her prize. Each Prize winner should consult his or her tax advisor about the tax implications of winning a Prize.
CONDITIONS OF PARTICIPATION: By participating, entrants agree: (a) to release Sponsor, its affiliates, subsidiaries, retailers, sales representatives, distributors, Marketing Lab Inc. and each of their respective officers, directors, employees and agents (“Promotional Parties”), from any and all liability, loss or damage incurred with respect to the awarding, receipt, possession, and/or use or misuse of any prize, (b) under no circumstances will entrant be permitted to obtain awards for, and participant hereby waives all rights to claim, punitive, incidental, consequential, or any other damages, other than for actual out-of-pocket expenses; (c) all causes of action arising out of or connected with this Sweepstakes, or any prize awarded, shall be resolved individually, without resort to any form of class action; and (d) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, excluding attorneys’ fees and court costs. Acceptance of prize constitutes winner’s consent to use his/her name, likeness and biographical information for advertising/promotional purposes in any and all media now known or hereinafter invented without territorial or time limitation
strong>GENERAL: If for any reason this Sweepstakes is not capable of running as planned, Sponsor reserves the right to cancel, terminate, modify or suspend the Sweepstakes or any portion thereof. In the event this Sweepstakes is cancelled, Sponsor reserves the right to conduct a random drawing to award all prizes from among all eligible, non-suspect, entries received prior to the time of the action or event warranting such cancellation. In no event will more than the stated number of prizes be awarded. Sponsor and its agents are not responsible for any errors, misprints or typographical mistakes in any printed or electronic advertising of the Sweepstakes. This Sweepstakes is governed by laws of the State of Colorado with venue in Denver, CO for the resolution all claims and disputes.
