Qwest Center for Customers with Disabilities (CCD)

Equipment can be purchased either from Qwest or from an outside vendor.

Equipment must match the disability.

Loan installment is billed as part of the regular telephone bill.

Loan amounts range from $50 to $1600

Customers may have several loans with Qwest, but the total loan amount cannot exceed $1,600.

Loan does not cover sales taxes on the purchase.

Loan does not cover shipping & handling.

Loan is interest-free.

Payments are collected in monthly installments ranging from $5 to $60 over a period of 3 to 60 months.

Customer's credit with Qwest must be satisfactory.

Application must be signed by a doctor or certifying authority
