Add Freeze

We make it easy for you to add a freeze to your Qwest® local or long-distance service.

E-mail

  1. Open the Letter of Authorization Form.
  2. Complete all requested information by typing it directly into the form.
  3. Save the completed form on your computer.
  4. Attach the completed form to an e-mail message and send it to picfreeze@qwest.com.

All requests received after 3PM CST will be processed the following business day.
