Qwest Moves Rates, Terms, and Conditions to Catalogs and Rate Schedules

The provisions governing many of the telecommunications services and features provided by Qwest Corporation, Qwest Communications Corporation, and Qwest LD Corp. to Wyoming customers moved from Price Schedules filed with the Wyoming Public Service Commission to Catalogs (Qwest Corporation and Qwest Communications Corporation) and Rate Schedules (Qwest Communications Corporation and Qwest LD Corp.) set out on the Qwest Web site, effective September 20, 2007.

As of September 20, 2007, as described in notices and fulfillment material provided to Wyoming customers, the Catalog Terms are the contract between Wyoming customers and Qwest Corporation/Qwest Communications Corporation for local telecommunications services and features and Qwest Corporation-offered long-distance services. The applicable Qwest Service Agreements (available at http://tariffs.qwest.com:8000/Qwest_Service_Agreements/LD_Xpl/index.htm) and applicable Wyoming Rate Schedules are the contract between Wyoming customers and Qwest Communications Corporation/Qwest LD Corp. for interexchange (long distance) and related services.

The Catalogs, Service Agreements and Rate Schedules are contracts between the relevant Qwest entity and the customer. By ordering, using, paying for, and/or not canceling the services or features, the customer agrees to the provisions in those documents. Initially, the rates, terms and conditions for the affected services and features are the same as they were in the Price Schedules. Over time, Qwest may change the Catalogs, Service Agreements, and Rate Schedules and will give you reasonable notice of important changes instead of filing them with the Wyoming Commission.

For further explanation of the changes, and to see Qwest’s Price Schedules, Catalogs, and Rate Schedules, please visit us at http://tariffs.qwest.com:8000/Q_Tariffs/WY/index.htm, or call us at 1 800-244-1111 (for residential customers) or 1 800-603-6000 (for business customers).
