Application Instructions
A classroom teacher or a team of classroom teachers who are employed by an eligible school district may apply for one CenturyLink Teachers & Technology grant by complying with the following steps. Grants may be considered ineligible if guidelines are not followed.
STEP 1
Be sure that your district is listed among the eligible school districts for this grant award. The list can be found here.
STEP 2
Complete the cover sheet. The cover sheet requires you to provide contact information for yourself, your school, and your school principal. The cover sheet also requires you to provide additional information that is relevant to your grant proposal. This document must be signed by you and your school principal.
Note: If a team of classroom teachers is applying for a CenturyLink Teachers & Technology grant, each team member must complete a separate cover sheet.
Note: The cover sheet shall be the only document that contains personal or identifying information about you, any of your teammates, your school, your school principal, or your school district.
STEP 3
Please prepare a narrative description of your grant proposal not to exceed four single-spaced pages. Your grant proposal must describe the goals and objectives you want to achieve. At a minimum, your grant proposal must:
- describe the different types of student populations and student grade levels that you will serve;
- provide the number of students that you will serve;
- clearly articulate how you plan to improve student achievement by the integration of technology into your classroom curriculum through innovative ways;
- clearly describe how you will measure student achievement gains as a result of integrating innovative technology strategies into the classroom curriculum;
- describe any additional outcomes you seek to achieve;
- describe how you will measure the success of these additional outcomes;
- provide a detailed timeline for the implementation of the grant.
Note: Do not include additional materials such as CDs, PowerPoint presentations, etc.
Note: Do NOT include any specific identifying information about you, your school, or your district in the narrative. It is acceptable to include descriptions of your school and its demographics.
Example:
Acceptable: Our school is an urban elementary school with approximately 60% of the students
who are English Language Learners.
Not Acceptable: At Central Valley High in the City Public Schools, we...
STEP 4
Please prepare a budget narrative that describes how you will spend the grant funds. At a minimum, your budget narrative must include the amount of the grant you are seeking and must clearly describe how the grant funds will be spent. Unknown costs associated with certain items that will be purchased with grant funds should be estimated as accurately as possible.
The budget narrative must also describe how the innovative technology strategies that are integrated into the classroom will be sustained over time once the grant funds are expended.
STEP 5
Submit an itemized budget spreadsheet that reflects all relevant expenditures detailed in the budget narrative required in Step 4.
STEP 6
Please submit four copies each of the cover sheet(s), narrative description of your grant proposal, the budget narrative, and the budget spreadsheet via regular mail, postmarked no later than October 15, 2012 to:

Application Review and Notification
The CenturyLink Teachers & Technology grants will be awarded through a competitive process. All proposals for a grant will be reviewed and evaluated by a selection committee of individuals who are education, business and civic leaders. Grants shall be awarded solely on the basis of the merit of each proposal. The scoring of each proposal shall be confidential and individual feedback will not be provided. Grants will be awarded to individual applicants or team leaders on behalf of their team.
A scoring priority will be given to proposals that include innovative, creative practices using technology in the classroom.
Selected proposals may be awarded all or part of the proposed budget.
All individual applicants and team leaders will be notified whether or not their respective proposals have been selected to receive a grant by April 13, 2012. Funds for 75 percent of the grant shall be awarded in April 2012. Funds for the remaining 25 percent of the grant shall be awarded after the recipient has submitted the expense report that is no later than October 15, 2012 (see the following Expenditure of Grant Funds).
Note: Identical applications will not be considered.
Note: Only one application per teacher is allowed whether submitted as an individual or as a team member.
Expenditure of Grant Funds
All grant funds must be expended during the 2012 - 2013 academic year. Any funds not expended during this timeframe shall be returned to PEBC by December 3, 2013. Grant recipients must submit an expense report postmarked on or before October 15, 2012.

Special Note:
Keep in mind we are looking for ideas that foster creativity and innovation in the classroom. Be original in
ideas of how the technology will be applied to improve academic achievement and improve the classroom experience. We are also
looking for ways that technology can support not only your classroom, but how it might capture the attention of your colleagues
and expand throughout your school.
