To protect you and your family we offer a comprehensive benefits package. Our plan starts with medical, dental and vision coverage, term life & accidental loss insurance; short and long term disability coverage, voluntary dependent term life insurance (spouse and child), healthcare and dependent care reimbursement accounts, and voluntary long-term care insurance (for you, your spouse, adult children, and your parents). Most of these plans offer you several options to allow you to choose the coverage that is right for you.

Qwest also offers a 401-K plan, which allows employees to contribute up to 18 percent of their pay with a 100% match on the first 3% contributed. Employees can begin contributing to the 401k Plan after receiving their first paycheck. The employer matching contribution begins after 12 months of service. The maximum contribution for 1999 is limited to $10,000.00 as determined by IRS regulations.

Additionally, Qwest has an Employee Stock Purchase Plan, which allows you to purchase stock at a discount from fair market value. Employees can begin participating after receiving their first paycheck.

Credit Union membership rounds out our benefits package.

Time Off with Pay
Qwest full-time employees and part-time employees (regularly scheduled to work 20 hours or more per week), are eligible for Time Off with Pay (TOP). Employees accrue TOP at a rate based upon the employee's length of service. Part-time employees' TOP is prorated based upon number of hours regularly scheduled to work.

Accrual Schedule

- 4 years of service: 20 days per year
- 14 years of service: 25 days per year
- 19 years of service: 30 days per year
+ years of service: 35 days per year

Additionally, Qwest observes 8 holidays per year.

All flex days can be taken in increments of full or half days and as little as two hours for doctor or professional appointments.
