MWDVBE companies that want to apply for potential business opportunities at Qwest should submit:

1. The company’s current MWDVBE Certification. To find out how to become certified, visit the Credential Verification page.
   
   
2. A completed Prospective Supplier Information Form (see link “Becoming a Qwest Supplier”) with a minimum of four appropriate United Nations Standard Product/Services Code (UNSPSC). The public library or bookstore will have the UNSPSC manuals which contain these codes or access UNSPSC Code Search.
   
   
3. Any descriptive material or brochures about your company. Please do not send product catalogs.
